We are pleased
to present detailed diabetes care data for Alliance Primary
Care physicians for 2005 and 2006. Explanations
of each indicator and how the data were gathered follow the
table. Only data for physicians with at least 10 eligible patients
as of April, 2006 are shown. Totals for each office and for
Alliance Primary Care as a whole are included.
These data demonstrate high (good) annual rates of testing
for HbA1C (a measure of blood sugar control), and for LDL-cholesterol.
Rates of achieving good levels of blood sugar and LDL-cholesterol
control vary among physicians but demonstrate significant improvements
from 2005 to 2006, especially for LDL-cholesterol.
Alliance Primary Care is highly committed to continuous improvements
in quality, safety, and service. Transparency – presenting
quality data publicly – is an important part of our quality
improvement programs. We believe that the depth and detail
of the data presented are unique in this region if not the
whole country.
